Program Guidelines
- Patient’s portion of the bill is due the day of the service
- There will be a $50 reinstatement fee if your plan lapses
- Cannot be used with another plan
- No refunds of premiums
- NON-REFUNDABLE
- No roll overs if not used within the 12 month period
Program Exclusion & Limits
- In conjunction with another dental plan, dental insurance, or financing
- For treatment which, in the sole opinion of our doctors, lies outside the realm of their capacity
- For referrals to specialists
- For hospitalizations or hospital charges of any kind.
- For cost of dental care which are covered under automobile medical.
- For services of injuries covered under automobile medical
